Fortis PCI Compliance Questionnaire

You will learn how to complete the Fortis PCI Compliance Questionnaire in Bravo to ensure secure payment processing and maintain industry compliance standards.

Go to https://pcismart.com and follow these instructions, or call the PCI Helpdesk at
(800) 567-8106, Monday – Friday 8 AM – 5 PM, CST for assistance.


• Login as first-time user with your full Merchant ID. If you get an error on the firsttime login, use the regular login option with the last 5 digits of your Merchant ID,
plus your state abbreviation in caps (i.e. 12345TX) as the passcode.
• Reset and record your new password. Your certificate must be renewed each year.
• Merchant Information, Part 1: Check your information for accuracy and edit as
necessary.
• Merchant Information, Part 2, Type of Merchant Business: Check E-commerce if
online through Buya.com only, and Retail if Fortis processes payments in your
stores(s).
• Merchant Information, Part 3, Relationships: Answer YES to both questions.

• Merchant Information, Part 4, Processing Solution: Select Moto/E-commerce.
• Merchant Information, Part 4, Processing Solution, question: Do you store any
sensitive cardholder data electronically? Answer NO.
• Second section, Under Moto/E-commerce: How do you process payments? Answer
Hosted Payment and iFrame
• Under Add Solution, in the drop-down menu select Fluid Pay, LLC, and then
Merchant Servicer; Third Party Servicer in the adjacent box.
• Agree; Save & Continue


• The next sections are a series of questions, most of which are not intuitive. The
Helpdesk advised to answer YES to all questions in all sections. If you choose any
other answer, you will not be certified!
• Review and sign your questionnaire.
• Save a copy of your certificate.
• Send a copy of your certificate to support-fl@fortispay.com
